Volunteer

Thank you for your interest in volunteering at The Bridge Avenue School. We appreciate you and your willingness to help our students. You are a vital part of our school.

In order to comply with TBAS policy and our insurance company requirements, all volunteers must complete:

  1. New Volunteer Application [DOC] or Returning Volunteer Application Form [DOC]
  2. ADP Disclosure Form [PDF]
  3. ADP Volunteer Release Authorization Form [PDF]

The application gives us information on your experience, interests, availability, and other background information. By signing the ADP Disclosure Form, you are allowing TBAS to obtain a background check from ADP. ADP will search a national sexual offenders database and other court records. The ADP Release Authorization Form releases the information obtained to TBAS.

If possible, we are also asking for a tax-deductible donation of $10.00 to defer the cost of the screenings. Also included in the Release Form, for your records, is “A Summary of Your Rights Under the Fair Credit Reporting Act” provided by ADP.
